Based on observation, interview and record review, the licensee did not comply with the section cited above in that sleep logs were not up to date for the 4 infants enrolled which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, they will continued documenting the infants sleep and have the logs available for depatrment review by POC due date of 8/11/2025.
Based on observation, interview and record review, the licensee did not comply with the section cited above in that staff # 3 and # 4 did not have proof of current mandated reporter training certicicate on file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Per Licensee, staff # 3 & # 4 will take training and proof of POC will be available for review.
