Based on observation, the licensee did not comply with the section cited above in 1 of 1 outdoor spaces with holes in the outdoor padding which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation and record review the licensee did not comply with the section cited above in 13 of 13 days of sign in/out sheets between 4/24/24 - 5/13/24 where at least one child on each day that was signed in but not signed out of the facility which poses/posed a potential health, safety or personal rights risk to persons in care.

LPA observed children's snack items, in dry storage room, Goldfish and Cheezits, to be opened from their original containers and not sealed nor labeled with expirations. This poses a potential risk to the health and safety of children in care.

LPA observed expired food in each chiild's Emergency bag. LPA also observed a non- working mini- refrigerator to have string cheeses, Go-Gurt, relish, lemonade, and mold inside, and accessible to children. This poses a potential health and safety risk to the children in care.

(20) All equipment (fixed or mobile), dishes and utensils shall be kept clean and maintained in safe condition.

Based on observation, the licensee did not comply with the section cited above as LPAs observed a cart in the food storage closet with 2 pans and a pot with dried food on them from a prior day which poses/posed a potential health, safety or personal rights risk to persons in care.

Licensee did not secure fencing around outdoor play area. LPA observed an opening on left end side of the fencing, and 2 wood pieces barricading the opening, allowing for a 9 inch and 7 inch gap surrounding wood pieces. This poses a potential health and safety risk to the children in care.

Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 1 disaster drills not conducted every 6 months, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in 1 out of 4 staff without fingerprint clearance which poses an immediate health, safety or personal rights risk to persons in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above as Child #1 did not have any file or records at the facility, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on observation the licensee did not comply with the section cited above as the food storage area (closet) in Room 117 was observed to have bleach, hand soap, disinfectant wipes, toilet bowl cleaner, and cans of paint on the shelf and the door to the food storage closet to be unlocked, which poses/posed a potential health, safety or personal rights risk to persons in care.

Per director, I noticed the carpet needs to be cleaned. I noticed sometimes the sinks needs to be cleaned. Its the teachers that use them all the time. LPA observed the bathrooms toilets in the rooms to have a yellow ring stains inside/outer part of the the toilets. This poses a potential risk to the health and safety of children in care

Based on observation, the licensee did not comply with the section cited above as LPAs observed 2 pizza boxes on top of the pans and pot in the food storage closet. The top pizza box had an egg carton and a half filled yogurt container with a metal spoon inside the pizza box, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in 4 of 7 children’s files reviewed to be missing the LIC 701 Physicians Report, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in 1 out of 4 staff files reviewed to be missing proof of current mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Upon arrival LPA observed 13 children being supervised by Teacher Ms. Torres. Three additional children arrived. Total children being supervised by one teacher was 16 children. This poses an immediate risk to the health and safety of children in care.
