Based on observation and interview, the licensee did not comply with the section cited above in that Licensee has not purchased a life ring available which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will buy life ring and have it available for department review by POC due date of 7/4/2025

Based on observation and interview, the licensee did not comply with the section cited above in that there was no daily log available for review which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will begin daily log and have it available for review by poc due date of 6/4/2025

Based on observation, the licensee did not comply with the section cited above in that child # 1 was sleeping in car seat on top of a table which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Child was placed in crib during inspection- Per Licensee they will submit a written plan to remian in compliance by POC due date of 6/18/2025

Based on observation and interview, the licensee did not comply with the section cited above in that LPA observed alarm still in box and not set up in the pool which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will set up the alarm and have it available for department review by POC due date of 7/4/2025

Based on observation and interview, the licensee did not comply with the section cited above in that Licensee did not purchase rescue pole which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee, they will buy rescue pole and have it available for department review by POC due date of 7/4/2025

Based on record review, the licensee did not comply with the section cited above in 1 out of 3 staff (Staff #2) without current Mandated Reporter Training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will have Staff #2 take the Mandated Reporter Training and place a copy of the training certificate in employees file.

Based on observation, the licensee did not comply with the section cited above in 1 of 1 pool gates which are not self-closing or self-latching which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will repair the pool gate to ensure it is self-latching and self-closing.
