Based on interview and record review, the licensee did not comply with the section cited above in 4 out of 5 staff files reviewed without proof of measles immunization and 2 of 5 staff files without proof of pertussis immunizations, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in 6 out of 6 staff files reviewed that did not have proof of current mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

(1) A result with values of 5.5 ppb or greater shall be deemed an Action Level Exceedance. This requirement is not met as evidenced by Based on record review, the licensee did not comply with the directive above, as one outlet out of nine outlets tested with an Action Level Exceedance (ALE). This poses an immediate Health and Safety risk to children in care.

Staff #1 did not have fingerprint clearance in Guardian at the time of visit.
