Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 3 staff with no proof of mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have Staff #2 complete Mandated Reporter Training and place proof of completion in facility file for Staff #2.

Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 3 staff with no proof of measles, pertussis immunizations and proof of negative tuberculosis test, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have Staff #1 obtain proof of measles and pertussis immunizations and negative TB test (within one year of start date)and place proof of measles and pertussis immunizations and negative TB test (within one year of start date) in facility file for Staff #1.

Based on interview and record review, the licensee did not comply with the section cited above in 1 out of 1 providers or staff without current Mandated Reporter Training, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will take the Mandated Reporter Training from an approved training site and email LPA the completed Mandated Reporter Training Certificate.

Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 2 infants in care with no completed LIC 9227 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will complete the LIC 9227 and place in both infants files, and email a copy to LPA.

Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 2 infants without a Sleep Log documenting their sleep during 15 minute checks which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will begin checking on sleeping infants every 15 minutes and documenting the date and time of the infants sleep checks.
