LPAs observed from 9:40AM-12:00PM 78 out of 78 children and 9 staff were in the chapel, which is not within licensing conditions. Per record review, facility sketch chapel is not included on the license. Per license, "...in rooms peace, wisdom, vision, joy, faith, & hope. LPA interview w/licensee.

Based on interview and record review, the facility did not comply with the section cited above. LPA was not able to verify if lead testing was completed on website: State Water Resource System. Director was unable to access Lead Testing documentation during todays inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.
