Based on observation, interview, and record review, LPA observed the lockable device no lower than 60 inches from the ground. Staff 2 opened the gate and the gate did not self close or self latch correctly which poses an immediate risk to children in care.

Resolution: Licensee will repair the gate to ensure it is self closing and self latching.

Based on record review, Staff 3 (S3) did not have current crimincal record clearance or child abuse index clearances which poses a potential health and safety risk to children in care. According to licensee, it was S3's first day and they were going to get fingerprinted that day.

Resolution: At 8:45am, S3 left the facility. S3 will not return to the facility unless fingerprint cleared. LPA provided a request for livescan form.

Based on record review, facility does not have documentation of fire drills which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct and document fire drills every six months and submit to LPA via email by 5pm on 12/4/25.

Based on observation, record review, and interview, the facility does not have a life ring, body hook, pool alarm or safety checklist which poses a potential risk to children in care.

Resolution: Licensee will purchase the following pool safety features; life ring, body hook, pool alarm and conduct a daily safety checklist to ensure compliance.

Based on record review, the facility does not have sleep check for the Child 4 (C4) which is 24 months and younger which poses a potential risk to children in care.

Resolution: LPA discussed safe sleep regulations and provided a copy of infant sleep log. Licensee will submit sleep logs for C4 for the dates 11/20/25-12/4/25 and submit to LPA via email by 5pm on 12/4/25.
