Based on observation, interviews, and review of Lead water testing samples taken on 12/21/2022 revealed that levels of lead were beyond allowable amount deemed by the State of California. Room 29 water fountain had a lead exceedance levels of sample "B" at 5.67 ppb. This poses a potential health and safety risk to children in care.

Based on observation and interview, the licensee did not comply with the section cited above in Epi Pen was observed in a zip lock bag in a locked cabinet without an original unaltered label. No prescription label was on the epi pen which poses/posed a potential health, safety or personal rights risk to persons in care.

(g) The licensee shall document each child's immunizations and shall maintain such documentation in the center for as long as the child is enrolled.
