LPA observed tiles on the ceiling in several of the class rooms and hallways to the class rooms had stanied water damaged and some tiles hanging down from the roof from rain buildup. LPA also observed a couple of toilets not working properly (Flusing slowly). This poses a potential risk to the health and safety of children in care.
Interviews and documents reviewed and received showing, During nap time one teacher was alone with the 20 children and a few of the children were awake and were not able to sleep. This poses an immediate risk to the health and safety of children in care.
, around August 2022, staff #2 pulled a child's hand out of staff #1 hand and when doing that staff #1 hand flew and hit a door and staff #2 yelled at staff #1 in front of a child.
All toilets, handwashing and bathing facilities shall be maintained in safe and sanitary operating condition.
