Based on (record review), the licensee did not comply with the section cited above infant in care did not have a 15 minute log which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee will begin to maintain 15 minute log and provide copy by POC due date.

Based on (record review), the licensee did not comply with the section cited above in 1 out of 6 staff files did not have TDAP and MMR vaccines in the file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee will provide verification of TDAP and MMR for staff #1 by POC due date.

Based on (record review), the licensee did not comply with the section cited above in 1 out of 1 infant file did not have LIC 9227 for infant in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee will provide copy of LIC 9227 by POC due date

Based on observation, interview, and record review, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. LPA observed 17 children in care at one time on this date. Licensee is only licensed for 14 children at one time.

Resolution: Licensee states she will ensure she does not go over her 14 capacity at one time and will send the Department a list of each child's name that is enrolled, with the days and hours each child is in care on a regular bases.

Based on LPA observation, interviews and record review, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care. Licensee assistant was alone caring for 14 of the enrolled day care children in an unlicensed address, next door from the day care.

Resolution: Licensee states will cease operating her day care out of the Unlicensed home next door to her day care. Licensee states will write a declaration in detail, on how she will ensure she will not use the unlicensed home next door to conduct her day care.

Based on observation and interview conducted, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee states she will ensure she follows the regulation of the Conduct which is inimical to the health, morals, welfare, or safety of either an individual in or receiving services from the facility or the people of this state, by being truthful and forth coming regarding the information requested by the department at all times. Licensee states she will write a declaration regarding usage of the unlicensed home, her over night care, fingerprint of adults and all the children enrolled.

Resolution: Licensee states her and her assistants will take the mandated reporter training and submit a copy of each of their certificates by the POC date.

Resolution: Licensee states her and her staff will privde proof of Pediatric First Aid and CPR by the POC date.

Based on interview, the licensee did not comply with the section cited above in 2 counts which poses an immediate health, safety or personal rights risk to persons in care. Licensee has 2 assistants without fingerprint clearance

Resolution: Licensee states one of her assistant no longer works for her and the other assistant she will have fingerprint cleared as soon as possible and will submit proof to the department.

Based on LPA observation the licensee did not comply with the section cited above in 1 count which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: 102417(g)(5) - Licensee has several small water fountains around the back yard with water in them. Licensee states will remove water from all fountains and replace with rocks or plants. Licensee will send photos of the entire back yard where all the water fountains are located.
