Based on observation, the licensee did not comply with the section cited above LPA observed the carbon monoxide detector not working properly may fail or give false alarm due to sensor aging which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit proof of purchasing a new carbon monoxide detector along with a picture to the department by POC due date 12/29/2025.

Based on record review, the licensee did not comply with the section cited above. Licensee did not have a current Mandated Reporter training certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a current Mandated Reporter certificate to the department by POC due date 12/29/2025. Training can be found at www.mandatedreporterca.com.

Based on record reviewe, the licensee did not comply with the section cited above. Licensee did not have immunization record against influenza on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit immunization against influenza for herself to the department by POC due date 12/29/2025.

Based on record review, the licensee did not comply with the section cited above. Child 1 is missing LIC995 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC995 for child 1 to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Child 3 is missing immunization records and LIC9227 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit LIC9227 for child 3 to CCLD by POC date.

Based on observation, the licensee did not comply with the section cited above. LPA observed home is under construction. Licensee is having her kitchen re-done and extending the kitchen and master bedroom. Licensee stated she did not inform licensing of the changes she was making to the home which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee's husband showed LPA permit for construction and plans for the home. LPA requested once construction is complete that she submit a new facility sketch to the Department.

Based on observation, the licensee did not comply with the section cited above. LPA observed two guns stored in another off limits area not in a locked safe and not easily accessible to children. One gun did not have a trigger lock which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee's husband moved the guns to a locked safe and understands they must be locked or have a trigger lock on them at all times.

Based on observation, the licensee did not comply with the section cited above. LPA observed three guns in a locked safe in an off limits area with bullets stored in the same safe which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee's husband removed the bullets and stored them in a locked storage away from the guns. Deficiency cleared today.

Based on observation, the licensee did not comply with the section cited above. LPA observed carbon monoxide detector was not plugged into the wall which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee plugged it in during visit. Deficiency cleared today.

Based on observation, the licensee did not comply with the section cited above. LPA did not observe a fire and disaster drill log which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a fire and disaster drill and submit an updated log to CCLD by POC date.

Based on interview and record review, the licensee did not comply with the section cited above. LPA discussed SB792 Immunization Requirements and observed licensee did not have immunization records on file. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit immunizations against pertussis, measles and influenza for herself to CCLD by POC date.

Based on interview, the licensee did not comply with the section cited above. Licensee states she does not have a Safe Sleep log for infants which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LPA explained Safe Sleep regulations to licensee. Licensee stated she understood and will begin to keep a Safe Sleep log for infants.
