Licensee is having her kitchen re-done and extending the kitchen and master bedroom. Licensee stated she did not inform licensing of the changes she was making to the home which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee's husband showed LPA permit for construction and plans for the home. LPA requested once construction is complete that she submit a new facility sketch to the Department.

Based on observation, the licensee did not comply with the section cited above. LPA observed two guns stored in another off limits area not in a locked safe and not easily accessible to children. One gun did not have a trigger lock which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee's husband moved the guns to a locked safe and understands they must be locked or have a trigger lock on them at all times.

Based on observation, the licensee did not comply with the section cited above. LPA observed three guns in a locked safe in an off limits area with bullets stored in the same safe which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee's husband removed the bullets and stored them in a locked storage away from the guns. Deficiency cleared today.

Resolution: LPA explained Safe Sleep regulations to licensee. Licensee stated she understood and will begin to keep a Safe Sleep log for infants.
