Based on LPA's observation, the fire extinguisher was last serviced on 12/2022 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states that Fire Marshall will service the existing fire extinguisher at the entrance of the day care and will submit proof that fire extinguisher has been service reflecting the current date by 01/17/25.

Based on record review and licensee statement, P1 is residing in the home without fingerprint clearances which poses an immediate health, safety or personal rights risk to persons in care. Civil penalty of $500.00 was assessed.

Resolution: By POC due date: 01/09/25, licensee to submit a written plan to ensure that all adults residing or working in the home shall have fingerprint clearances prior to initial presence in the home.

Based on LPA's record review, the licensee did not comply with the section cited above in three out of three persons, Licensee and two Assistants which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide proof of documentation of current Mandated Reporter Training for Child Care Providers by/29/25

Based on observation, interview, and record review, the licensee did not comply with the section cited above for two staff members, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The Licensee will submit current certificate for Mandated Reporter training- Child Care Providers (AB1207) to the Department by 8/18/2023. If taking training in person, proof of registration to be submitted if occurring after due date. Online can be taken at www.mandatedreporterca.com

Based on observation, interview, and record review, the licensee did not comply with the section cited above for one infant in care, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The Licensee will submit copy of nap check documentation for one (1) infant in care. The Licensee will ensure the crib meets all requirements for Safe Sleep including no blankets, pillows, or other items in the crib with the infant. Proof of documented nap checks to be submitted to the Department by 8/18/2023.

Based on observation, interview, and record review, the licensee did not comply with the section cited above for three children in care, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: The Licensee will submit signed copies of LIC9224 and provide copies of LIC809 to parents regarding office visit held on 6/26/2023. Copies of signed LIC9224 shall be submitted to the department by 8/10/2023.
