Based on observation, interview and record review, the licensee did not comply with the section cited above in Child 2 was observed sleeping inside a white bassinett in the living room area, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LIcensee removed napping child from bassinet during site visit, Licensee will submit a written plan of her understanding CCR 102425 to the San Jose Regional Office by close of business 10/03/25.

Based on observation, interview and record review, the licensee did not comply with the section cited above in 2 plastic packets of pledge wipes and ant traps were accessible in the on limit kitchen cabinet, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee removed accessible items during site vist, Licesee will submit a written plan on measures to prevent deficiency from repeating, to the San Jose Regional Office by close of business 10/03/24.

Based on observation, interview and record review, the licensee did not comply with the section cited above in, play pen with her infant grandchild does not have a mattress, blanket was used for a fitted sheet, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee will send a wriiten response to her understanding of CCR 102425(a)(3) to the San Jose Regional Office by close of business 10/16/24.

Based on observation, interview and record review, the licensee did not comply with the section cited above in Child 1 napping with a loose blanket and pillow in a playpen, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: LIcensee will submit a wriiten statement of her understanding of CCR 102425(b) by close of business to the San Jose Regional Office 10/16/24.

Based on observation, interview and record review, the licensee did not comply with the section cited above in Child 2 does not have LIC 9227 on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit a copy of completed LIC 9227 to the San Jose Regional Office for C2 by close of business 10/16/24.

Based on observation, interview and record review, the licensee did not comply with the section cited above in Child 1 and C2 do not have sleep logs, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit copies of completed safe sleep logs for C1 and C2 to the San Jose Regional Office by close of business 10/16/24.
