Based on observation, interview and record review, the licensee did not comply with the section cited above in stairs were not barricaded upon LPA arrival and during site visit, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee closed off and barricaded the stairs during site visit. Licensee will submit a written of her understanding of CCR 102417(g)(3) and preventive measures by close of business 07/12/24.
Based on observation and interview, the licensee did not comply with the section cited above in stairs were not barricaded when LPA arrived and during site visit, upstairs laundry room had detergent on the floor, upstairs bathroom had shampoo accessible, which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Licensee barricaded the stairs during site visit making all room in second floor inaccessible.
Based on observation, interview and record review, the licensee did not comply with the section cited above in, safe sleep documentation only shows nap time, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee was provided a sample of safe sleep log, licensee will submit copies of sleep logs for the three infants in care to the San Jose Regional Office by close of business 07/25/24.
