Based on observation, interview, record review, the licensee did not comply with the section cited above in, day-care room was built without city permits, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will submit proof of applying for city permits for the attached day-care room, licensee will also cease using the un-permitted room for day care children. Proof of permit process will be sent to the San Jose Regional Office by close of Business 10/11/2024.
Based on observation, interview, and record review, the licensee did not comply with the section cited above in, door to garage partially open, detergents on top of washer, which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee immediately closed the attached garage door during site visit, licensee will submit written statement of her understanding CCR 1024179(g)(4)(A) and efforts to keep deficiency from occurring to San Jose Regional Office by close of business 10/04/24.

Resolution: Licensee will submit proof of city final inspection of porch construciton to the San Jose Regional Offie by close of business 10/11/24.
