Based on observation, the licensee did not comply with the section cited above. LPA observed a fully charged 1A10BC fire extinguisher and an empty 3A40BC fire extinguisher which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have fire extinguisher serviced and submit proof to CCLD by POC date.

Based on observation and record review, the licensee did not comply with the section cited above. LPA observed a fire and disaster drill log which was last completed on 01/19/2024 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will conduct a fire and disaster drill and submit updated log to CCLD by POC date.

Based on record review, the licensee did not comply with the section cited above. Child 1's immunization records need to be updated which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will submit updated immunization records for child 1 to CCLD by POC date.

Based on record review the licensee did not comply with the section cited above. Three of twelve children are missing immunization records in their file; which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to practice and document a fire disaster drill and send to LPA by plan of correction date.

Based on observation and record review the licensee did not comply with the section cited above. Licensee and assitants (Angel Ortiz and Mariana Vasquez) do not have an updated mandated reporter training available for review. Which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit copies of Mandated Reporter Training for child care providers for herself and two assistants by plan of correction date.

Based on record review, the licensee did not comply with the section cited above. Licensee does not have current cpr & first aid certifications available for review. Which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee took her cpr & first aid certification online. Licensee agrees to submit proof of completion of emsa approved cpr & first aid. Course is to be taken in person.

Based on record review the licensee did not comply with the section cited above; Licensee's assistants did not have the LIC 508 criminal record statement in their file available for review which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit copies of staff files to LPA including LIC 508.

Based on record review the licensee did not comply with the section cited above. Licensee does not have immunizations for assistants (Angel Ortiz and Mariela Vasquez) available from review which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submitt proof of Immunization for assitants by plan of correction date.

Based on record review; children 8 and 9 are missing the LIC 627 in their file.

Resolution: Licensee agrees to update children files and submit copies to LPA by plan of correction date.

Based on observation, the licensee did not comply with the section cited above. LPAs observed two walkers in plain view, stored hanging from a rack in proximity to the ceiling. which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: On this day. licensee removed the items (two walkers) in the presencse of the LPAs, placed outside of the curtilage area of the side yard. Deficiency cleared.
