Based on observation the licensee did not comply with the section cited above. Carbon monoxide detector was not installed, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee had a new carbon monoxide/smoke detector which she installed during today's inspection. Deficiency is cleared today.

Based on observation and record review, the licensee did not comply with the section cited above Licensee's assistants' (Deli Martinez and Valeria M. Zaragoza) does not have current mandated reporter training; which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit copies of completed Mandated Reporter Training for both Deli and Adriana by plan of correction date of 06/01/22.

Based on record review the licensee did not comply with the section cited above. Two infants (#2 and #10) do not have an Individual Sleeping Plan (LIC9227) completed which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to provide parents with the LIC 9227 to complete and Licensee will complete her portion of the LIC 9227. Licensee to submit copies to LPA Berumen by plan of correction date of 06/01/22.

Based on record review, the licensee did not comply with the section cited above. Two infants (child #2 and #10) are missing the LIC 9227 in their file which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to file the completed LIC 9227 for child 2 and 10 and submit copies to LPA Berumen by plan of correction date of 06/01/22.
