Based on observation, interview, record review, the licensees did not comply with the required mandated reporter training due every two years as both certificates were expired, which poses a potential health and safety risk to persons in care.

Based on observation and interview, the licensee did not comply with the section cited above for outside play area where parrots/birds are located in cages that are accessible to children which poses a potential health, safety or personal rights risk to persons in care. Cages pose a risk as children can place their fingers inside of the cages in their current position.
Resolution: Licensee will submit photographs of parrot/bird cages made inaccessible to children by 6/6/2022.
