Based on observation, interview, and record review, the licensee did not comply with the section cited above for one (1) out of three (3) staff present, which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above as the former Site Director, Amy Rather, is no longer with the facility and was not reported to the Department, which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and interview, the licensee did not comply with the section cited above for outside playground structure that needs cleaning/stair rust repair and electrical outlets with no covers inside the facility, which poses a potential health, safety or personal rights risk to persons in care.

After water lead testing was completed, the facility had one faucet (F), located in the outdoor preschool play area, with an Action Level Exceedance of 61.4 ppb, which poses a potential risk to the health, safety, and personal rights of children in care.

Responsibility for Providing Care and Supervision- Care and Supervision. No child(ren) shall be left without the supervision, including visual observation, of a teacher at any time except as specified in sections 101216.2(e)(1) and 101230(c)(1). This requirement was not met as evidence by: a self reported incident occurred on 11/13/20. A child walked home without staff knowledge, parent called facility to notify the center. This pose an immediate risk to health and safety of children in care.

(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents.
