Based on interview and record review, the licensee did not comply with the section cited above. Licensee stated she does not have safe sleep log and does not document sleep check which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will check on sleeping infants and document on safe sleep log. Licensee will submit two weeks of safe sleep log to Fresno Community Care Licensing Office by 10/22/2021.
Based on record review, the licensee did not comply with the section cited above. Licensee completed her mandated reporter training on 08/27/2019 and has not completed her renewal training every two years following the date on which she completed the initial mandated reporter training. Assistant Tawni McCarty has not completed initial mandated reporter training which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete renewal mandated reporter training. Licensee will have assistant Tawni complete mandated reporter training on www.mandatedreporterca.com and submit both certificates of completion to Fresno Community Care Licensing Office by 10/22/2021.
Based on record review, the licensee did not comply with the section cited above. Licensee's pediatric cardiopulmonary resuscitation and pediatric first aid expired on 02/09/2021 which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will complete First Aid/CPR training and submit certification of completion to Fresno Community Care Licensing by 11/05/2021. If Licensee's appointment is after 11/05/2021, Licensee will submit verification of appointment to Fresno Community Care Licensing by 11/05/2021.
