Based on records reviewed and interviews, Staff 2 completed initial mandated reporter training on 02/16/2023 and has yet to complete renewal mandated reporter training every two years following the date on which he or she completed the mandated reporter training. Staff 2 has not completed renewal mandated reporter training, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will provide proof of completion of Staff's 2 renewal mandated reporter certificate by 01/16/2026.
Based on observation and interview, the licensee did not comply with the section cited above. LPA observed unlocked chemicals and cleaning supplies underneath the kitchen and hallway bathroom cabinet, which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee immediately removed the cleaning chemicals and supplies from underneath kitchen sink. Licensee immediately locked the hallway bathroom sink cabinet.
