Based on observation, interview, and record review, the licensee did not comply with the section cited above. The licensee could not provide a copy of the mandated reporter certificate which poses a potential health, safety or personal rights risk to persons in care.

Resolution: LPA will conduct an announced plan of correction visit.

Based on observation, interview, and record review, the licensee did not comply with the section cited above. Licensee's CPR/ First aid expired December 12, 2025 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee has an appointment scheduled on 02/28/26. LPA will conduct an announced plan of correction visit.

Based on interview, the licensee did not comply with the section cited above. Licensee could not produce proof of required immunizations (Pertussis, Measles, Flu). LPA informed that Flu vaccine is required annually, but is optional, so long as the individual in question provides a written statement (signed and dated) opting out of Flu shot for given year which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will get documentation to show proof. LPA will conduct an announced plan of correction visit.

Based on observation , the licensee did not comply with the section cited above. L. Cabrera observed dog feces in the living room where the children were watching television. LPA observed dog feces in the back yard, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately removed the feces and disinfect the area. POC Cleared.

Based on observation and interview, the licensee did not comply with the section cited above. Facility is a two-story home and stairs were not fence or barricaded, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately placed the fence and reported her daughter-in-law removes it. LPA reviewed regulation with Licensee. Stairs are to be fenced or barricaded when children under age 5 years old are present, Licensee agreed.
