Based on record review, the licensee did not comply with the section cited above in LPA observed Child #1 lack form which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee will correct and submit copy to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above in LPA observed 3 month old infant napping in playpen on his back. which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately placed infant on his back. Per licensee, from now on will always place infant to nap on their back. Deficiency corrected during visit.

Based on observation and interview, the licensee did not comply with the section cited above in LPA observed fire extinguisher is fully charged but has not been serviced annually as required which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, will purchase a new one and submit proof to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above in LPA observed stairs was not barricaded upon entry which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately installed barricade at bottom of stairs. Per licensee, stairs will be barricade at all times when children are in care. Deficiency corrected during visit.

Based on observation, the licensee did not comply with the section cited above in LPA observed cleaning agents and other potentially dangerous items in kitchen cabinet under sink and bathroom cabinet under sink accessible to children which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately installed safety latch on cabinets. Per licensee, safety latch will always be in placed when children are in care to ensure compliance. Deficiency corrected during visit.

Based on record review, the licensee did not comply with the section cited above in drill log not available for review which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, will conducted drills and submit copy of drill log to LPA by POC due date.

Based on observation, the licensee did not comply with the section cited above in LPA observed blankets inside and hanging on playpen where infant was napping which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee immediately removed blankets. Per licensee, will ensure no loose items in or around playpen with napping infant to ensure compliance. Deificiency corrected during visit.

Based on record review, the licensee did not comply with the section cited above in Child #1 lack medical consent form LIC 627, and Child #3 lack Identification and Emergency Information LIC 700 and LIC 627 which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee will correct and submit copy to LPA by POC due date.

Based on [(observation) the licensee did not comply with the section cited above in the fire extinguisher has not been serviced since 2017, but it is still fully charged. Smoke detector, when tested has a faint sound not a loud beep, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per licensee, will purchase new fire extnguisher and install new smoke detector today and will submit corrections to LPA by 9/2/22.
