Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above. S1 who is not fully qualified and does not have criminal record clearance, was supervising 2 uncleared Aides (S2, S3) in the outdoor playground. There were 25 children present which put them all out of ratio which poses an immediate health, safety or personal rights risk to persons in care.

Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above. Director states facility did not get water lead testing done yet. This poses/posed a potential health, safety or personal rights risk to persons in care.

Based on interview and record review, the licensee did not comply with the section cited above by not ensuring at least one staff member was trained in pediatric cardiopulmonary resuscitation and pediatric first aid during day care hours, which posed a potential health, safety or personal rights risk to persons in care.

(1) A licensed child day care center, as defined in Section 1596.76, that is located in a building that was constructed before January 1, 2010, shall have its drinking water tested for lead contamination levels on or after January 1, 2020, but no later than January 1, 2023, and every five years after the date of the initial test.

(e)(1) Criminal Record Clearance (e) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility: (1) Obtain a California clearance or a criminal record exemption. This requiremernt is not met as evidenced by: Staff CALISSA HANSEN has been working at the center since May 2023, does not have fingerprint clearances and is not associated to the license. This is a repeat violation within 1 year and poses a potential risk. Civil penalty of $250 is issued today.
