Two staff files are missing LIC503 Health Screening Report and record of TB. Based on record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.

Through interview it was found that a staff member was yelling at children and using a harsh tone when speaking to them which is a potential risk to the health, safety and personal rights of the children in care.
Based on Lead Testing Sample results the facility has at least 1 water faucet with lead exposure. This is a potential risk to Health and Safety or Personal Rights risk to persons in care.
