LOPEZ FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. Licensee had one infant in a pack and play wrapped in a blanket.

Resolution: Licensee will discuss with mom the dangers of the infant having a blanket in the pack and play while sleeping and will no longer allow the infant to use the blanket during sleep.

Based on observation, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. Licensee had two infants with blankets hanging over the crib.

Resolution: Licensee will no longer hang blankets over pack and play during naps. Licensee will provide a written statement that she has reviewed and will be in compliance with safe sleep regulation.

Based on LPA observation where C1 was seen in a crib swaddled with numerous blankets inside the crib. The licensee did not comply with the section cited above which poses posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee states that they will discontinue swaddling sleeping infants, and conduct in-service training with all staff. Licensee further states that proof of POC will be provided by 5pm on due date.

Based on observation, the licensee did not comply with the section cited about ofgarage door was unlocked making cleaning products in garage accessible to children in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee locked the garage door during visit and will sign a statement stating the garage door will remain locked making garage and it's contents inaccessible to children while in care.

Based on record review, the licensee did not comply with the section cited above in one out of three staff files did not have proof of Measles Mumps Rubella vaccine, (S2) which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have S2 receive immunization as soon as possible and provide proof to LPA Hamilton via text, email or USPS on or before the POC due date.

Based on record review, the licensee did not comply with the section cited above in two out of two children's (C1 & C2) records did not have the required LIC 9227 Individual Sleeping Plan which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have the LIC 9227 completed and signed by C1 & C2's parents and will provide proof to LPA via text, email or USPS on or before the POC due date.
