KLEIN, SUZANNE FAMILY CHILD CARE

Based on LPA's observation, the licensee did not comply with the section cited above in which seven children were present of whom three are under the age of two years old which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to submit in writing to Community Care Licensing how she will remain in compliance with Title 22 Regulations on ratio. Licensee reminded that a late plan of correction will result in a $100.00 for every day it is late.

Based on record review, the licensee did not comply with the section cited above in which there is no documentation of fire/disaster drill available for review during this inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct a fire/disaster drill and submit a copy of the documented drill to Community Care Licensing by 08/07/22. Licensee reminded that a late plan of correction will result in a $100.00 for every day it is late.

Based on record review, the licensee did not comply with the section cited above in which the assistant does not have immunization records (MMR/TDAP) available for review during this inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to obtain copies of assistant's immunization records and submit copy to Community Care Licensing by 08/07/22. Licensee reminded that a late plan of correction will result in a $100.00 for every day it is late.

Based on record review, the licensee did not comply with the section cited above in which an infant does have form LIC 9227 (Individual Sleeping Plan) in file for review during this inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to have form LIC 9227 placed in child's file and submit a copy to Community Care Licensing by 08/07/22. Licensee reminded that a late plan of correction will result in a $100.00 for every day it is late.

Based on record review, the licensee did not comply with the section cited above in which there is no docmented time chedk on infants in care for review during this inspection. which poses na potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to document a 15 minute check on infants and submit a copy of the documented form(s) to Community Care LIcensing by 08/07/22. Licensee reminded that a late plan of correction will result in a $100.00 for every day it is late.

Based on LPA's observation (photos taken), the licensee did not comply with the section cited above in which There is an above ground spa that is covered, however, all four locks are broken and the spa is full of water which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: LIcensee agrees to have spa cover locked or drain it by the end of business on 07/08/22. LIicensee reminded that late plans of correction will be assessed a $100.00 per day civil penalty for every day it is late.
