Based on interview and record review, the licensee did not comply with the section cited above where verification of fire drills was not available for review during inspection. Unavailable for review was proof of fire drills being conducted within the last six months or less. Which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email LPA a written decleration of understanding of 102417(g)(9)(A) and the requirement of documenting the drills for audit. Licensee will additionally email LPA a fire drill log and documentation of drill being conducted.

Based on observation, iinterview and record review, the licensee did not comply with the section cited above in two of three infants observed during nap time. Two of three infants utilized a sleep sack during naptime, without a medical exemption on file. C1 and C3 were both observed in a sleep sack,C1 was napping and C3 was awake. Which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email LPA either verification of a medical exemption for Child 1 and/or child 3 authorizing the appropriate use of a sleep sack or a written decleration of understanding, that sleep sacks are not permitted in licensed facilities without a medical exemption.

Based on interview and record review, the licensee did not comply with the section cited above in two of two staff files audited. Unavailable for review was documentation of required immunizations for S1 and S2. Which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email LPA proof of required immunizations for Staff 1 (S1) and (S2).

Based on interview and record review, the licensee did not comply with the section cited above in four out of five child files audited. Where either the required CDPH 286 was not available for review during inspection, blank or not updated. Which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will email LPA a written decleration of understanding regarding Title 22: 102418(g)(1). Additionally, Licensee will email updated CDPH 286 for C1, C3, C4 and C5. LPA provided LIC 857 Child Files Reveiwed, clarifying which child number corresponds to what documents are missing.

Based on interview and record review, the licensee did not comply with the section cited above as licensee is not docuementing 15-minute sleep checks, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will begin documenting sleep checks and provide proof to LPA.

Based on observations and record review, the licensee did not comply with the section cited above. Licensee allowed an uncleared adult to reside in the facility, which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee sent the uncleared adult to obtain fingerprint clearance and provided proof of the completed livescan form.
