On or about 8/30/2021, a child in care tested positive for COVID-19 (see LIC809 dated 9/1/2021). Based on LPA’s observation and interviews conducted, even after a child in this program tested positive for COVID-19, some children are not wearing face coverings and staff did not encourage children to wear face coverings upon entry or in the classrooms as required. This poses an immediate threat to the health and safety of the children in this facility. The preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED. An exit interview was conducted, and a copy of this report was reviewed with and appeal rights provided to Assistant Director Lizbeth Perez-Uribe whose signature on this form confirm the above-mentioned documents. A NOTICE OF SITE VISIT WAS ISSUED AND LPA VERIFIED THAT IT WAS POSTED IN A PROMINENT LOCATION AT THE FACILITY BEFORE LEAVING. THE LICENSEE UNDERSTANDS THAT IT MUST REMAIN POSTED FOR THE NEXT 30 DAYS ALONG WITH A COPY OF ALL TYPE A DEFICIENCIES (9099D) CITED DURING THIS INSPECTION. A COPY OF ALL TYPE A DEFICIENCIES CITED DURING THIS INSPECTION MUST ALSO BE IMMEDIATELY (within 24 hours of the child’s next day in care) GIVEN TO THE PARENTS OF ALL CHILDREN ENROLLED IN THE CHILD CARE FACILITY AND ANY CHILDREN ENROLLED INTO THE CHILD CARE FACILITY OVER THE NEXT 12 MONTHS.
