Based on interview and record review, the licensee did not comply with the section cited above in one out of five staff files reviewed. Unavailable for review during inspection was proof of health screening, inclluding TB clearance for Staff 2. Which poses a potential health, safety or personal rights risk to persons in care.

Based on record review four out of six staff records reveiwed did not have one or more rquired immunizations, which poses a potential health, safety or personal rights risk to persons in care.

Based on observation a perimeter gate in the playground was broken, leaving an opening large enough for a young child to exit to the parking lot of the facility which poses a potential health, safety or personal rights risk to persons in care.

Based on record reveiw, one child (Ref #5) was missing the medical assessment, TB and immunizations, which poses a potential health and safety risk to persons in care.

Based on observation a cabinet in one classroom had a sharp knife lying flat on the top shelf. The cabinet was not locked making it accessible to children in care which poses a potential health, safety or personal rights risk to persons in care.

Based on record review ten of ten staff did not have current mandated reporter training, which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in two of four staff files reviewed. Where Staff 3 (S3) was missing proof of TB clearance and S4 did not have a physical availbale for review. Which poses a potential health, safety or personal rights risk to persons in care.
