Based on record review, the licensee did not comply with the section cited above. Licensee and assistant #1 did not have current Mandated Reporter Training (AB1207) on file. Which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Per licensee, Mandated Reporter Training will be completed for licenee and assistant and submitted to CCLD by POC date of 03/28/2025.
Based on record review, the licensee did not comply with the section cited above in one assistant did not have her immunization records on file for MMR and Tdap which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that she will obtain a copy of staff #1's immunization records and put them in staff #1 employee file by the above poc date. Licensee stated that she will send a picture of staff #1 immunization record to LPA by the above poc date via text message.
Based on record review, the licensee did not comply with the section cited above in S1 did not have a personel file which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee states that S1 will have a complete personnel file on the above POC date and will send either by email or text messages neccessary forms.
