Based on record review, the licensee did not comply with the section cited above. LPA observed that Licensee did not have a valid Mandated Reporter Training Certificate which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee is to complete Mandated Reporter Training and provide the Department with a copy of their valid completion certificate.
Based on observation, the licensee did not comply with the section cited above as during today's inspection, LPAs observed one container of anti-freeze, six containers of motor oil, and one container of used motor oil that did not contain a top, located ground-level, in an accessible area of the backyard. This poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Staff #2 removed all motor oil and anti-freeze containers during today's inspection and placed them in the inaccessible garage. Deficiency cleared during inspection.
