Based on observation, the licensee did not comply with the section cited above as bottles of cleaning chemicals were located in a cabinet where the children use the accessible bathroom. Licensee advised the latch on the cabinet was broken which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee immediately removed the cleaning chemicals from the cabinet and advised she would place a new latch on the cabinet door by the end of the day.
Based on records review, licensee did not comply with the section cited above, as today, she was unable to provide proof of completion of a fire & disaster drill within the past six months. This poses a potential health, safety, or personal rights risk to persons in care.
Resolution: Licensee stated that she will conduct and document a fire & disaster drill by 3/24/2023. Licensee stated she will submit a copy of the completed log to the Fresno Community Care Licensing Office by 3/24/2023.
Based on observation, the licensee did not comply with the section cited above as during today’s inspection, LPA observed Infant #1 sleeping in a play yard with a small blanket lying beside them. This poses a potential health, safety, or personal rights risk to persons in care.
Resolution: Staff #1 removed blanket from play yard when asked to do so by LPA. LPA reviewed safe sleep regulations with Licensee & Staff #1. Licensee and Staff #1 stated they will not place any loose items (toys, bottles, blankets, pillows) inside any crib or play yard while infants are napping. Deficiency cleared during inspection.
Based on record review, the licensee did not comply with the section cited above, as Infant #1's filed did not contain an Infant Sleeping Plan as required. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will ensure and Individual Sleep Plan (LIC C227) is completed and on file for Infant #1 by 3/24/2023. Copy of completed form on file is to be submitted to the Fresno Community Care Licensing Office by 3/24/2023.

Based on observation, the licensee did not comply with the section cited above, as today, LPA observed an infant bouncer with the dining room of the facility. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: During today’s inspection, the licensee removed the infant bouncer. LPA provided Licensee with safe sleep information. The licensee understands that infant bouncers are not permitted for usage in family child care homes. Deficiency cleared at visit.
Based on record review the licensee did not comply with the section cited above in 2 out 5 children's files reviewed which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will obtain an emergency information for Child #1 and Child #2 and maintain proof within the family child care home by 5/31/22. An unnanounced plan of correction inspection will be condcuted after 5/31/22 by a Department representative to ensure clearance of this deficiency.
Based on records review, the licensee did not comply with the section cited above, as she was unable to provide proof of measles and whooping cough immunization for herself. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will obtain proof of measles and whooping cough vaccination and maintain proof within the family child care home by 5/31/22. An unnanounced plan of correction inspection will be condcuted after 5/31/22 by a Department representative to ensure clearance of this deficiency.
Based on record review, the licensee did not comply with the section cited above. During today's inspection, LPA found that although Adult #1 has obtained DOJ and FBI clearance through the live scan process, he has not obtained Child Abuse Central Index clearance which is also required. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated that Adult #1 will undergo live scan processing tomorrow. Licensee to submit proof of live scan to the Licensing Department by 5/6/2022. Licensee was advised that Adult #1 is not permitted to assist with day-care children until he is fully fingerprint cleared by the Department.
Based on record review and interview with licensee, the licensee did not comply with the section cited above as she stated she had not completed a fire drill within the last six months. Additionally, Licensee did not have a fire and disaster drill available for review. This poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated she will conduct and document a fire and disaster drill by 5/31/22. Licensee to maintain proof of log completion on site. An unannounced plan of correction inspection will be conducted by a Department representative after 5/31/22 to ensure clearance of this deficiency.
