Based on record review the licensee did not comply with the section cited above. Licensee was unable to produce 15 min. sleep checks for two out of two infants which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she shall immediately documenting 15 minute checks and submit copies of sleeping logs to LPA by POC due date.

Based on record review, the licensee did not comply with the section cited above. Licensee did not maintain record of measels, pertussis and influenza for assistant which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she shall obtain immunization records for assistant and provide proof to LPA by POC due date.

Based on record review, the licensee did not comply with the section cited above as children's records were incomplete as described in LIC 809 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will update missing documentation in children's records. LPA will conduct a POC inspection to verify that corrections were made.

Based on observation the licensee did not comply with the section cited above as infant was sleeping in assistants arms which posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will actively place the infant in play yard as soon as infant falls asleep. Licensee shall submitt a written statement of understanding by POC due date.

Based on record review, the licensee did not comply with the section cited above aslicensee was unable to provide documentation for Mandated Reporter Training for licensee and assistant which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will provide proof of Mandated Reporter Training for herself and assistant.

Based on record review the licensee did not comply with the section cited above. Licensee was unable to provide LIC 9052, TB clearance, LIC 9108 for assistant which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she shall maintain a complete record for assistant. Licensee stated she will provide proof of above mentioned documents to LPA by POC due date.

Based on record review, the licensee did not comply with the section cited above. Licensee could not provide evidence for herself and her assistant of completed Mandated Reporter Training (MRT). This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to complete the MRT and submit copies of MRT certificates to the Fresno South licensing office by 6/5/2023.

Based on record review, the licensee did not comply with this section. Three of the six children present do not have immunization records on file. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will obtain the immunization records for the 3 children and submit proof to the Fresno licensing office by 7/5/2022.
