MADRIGAL, MARGARITA FAMILY CHILD CARE

Based on record review, the licensee did not comply with the section cited above. Seven of the 9 day care children do not have completed forms PM286 (blue card) in files. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to complete the form PM286 for each day care child not enrolled in school. Licensee is to notify the Fresno South licensing office when the 7 - PM286 forms are completed.

Based on LPA Marquez observation the licensee did not comply with the section cited above. Licensee installed an above ground swimming pool without prior licensing approval. The pool does not meet the 5 feet height requirement and is not fenced allowing windows and sliding glass door direct access. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: The backyard is to remain off limits while the above ground pool is filled with water. Licensee agreed to empty and remove the swimming pool. Licensee will submit photos to the Fresno South licensing office by 8/25/2023

Based on record review, the licensee did not comply with the section cited above. Licensee's adult son who resides in the home, and is present today, turned 18 years old and does not have a CA Criminal Record clearance. This poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licensee is to have her adult son fingerprinted thru LIVE SCAN immediately. LPA Marquez advised Licensee Madrigal that her adult son shall not be present in the home during day care hours until he receives a CA Criminal Record clearance. Licensee stated she understands.

Based on observation, the licensee did not comply with the section cited above. LPAs Marquez and Alvarez observed an unlocked laundry room with detergents accessible to children and an unlocked off limits bedroom accessible to children which poses a potential health, safety or personal rights risk to children in care.

Resolution: During the inspection, Licensee placed a spinner knob on the laundry room door and locked the off limits bedroom door. DEFICIENCY CLEARED DURING VISIT.
