Based on record review, the licensee did not comply with the section cited above. Licensee and S1 did not have a California EMSA approved or American Heart Association issued First Aid/CPR certificate. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to provide the Department a valid California EMSA approved, or American Heart Association issued First Aid/CPR certificate for themself and for S1.

Based on LPA observation the licensee did not comply with the section cited above. LPA observed smoke detector not working which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to replace smoke detector and ensure it functions properly.

Based on LPA observation, the licensee did not comply with the section cited above. LPA observed cracked floor tiles with sharp edges in the kitchen which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to have cracks in floor tiles sealed or cracked tiles replaced.

Based on record review, the licensee did not comply with the section cited above, as she has not completed a fire and disaster drill in approximately 10 months. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will conduct and document a fire and disaster drill as required. Upon completion, Licensee is to submit a copy of the log to the Fresno Community Care Licensing Office, by 12/21/2022.

Based on record review, the licensee did not comply with the section cited above in 4 out of 8 children's files reviewed. Four childrens' files were missing LIC 995 A (Notification of Parents Rights and LIC 9150 (Notification of Additional Children in Care). This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she had a required forms check off list that she was using for children's files but did not know why the forms were missing. A new check off list was provided to licensee for childrens file requirements and staff files. Licensee stated she would have parents of child #1, child #2, child #3 and child #4 complete the missing forms LIC 995A and LIC 9150 and send proof to the Department by POC date 2/23/2022.
