Based on observation, the licensee did not comply with the section cited above. LPA observed a sandbox covered with wet and torn rugs and a small child's slide with a broken frame which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee is to remove the torn rugs and contaminated sand from the sandbox, place new sand and have a waterproof cover installed on the sandbox or find an alternative use for the area that meets licensing requirements. Licensee is to also have small child's slide removed from the home.
Based on record review, the licensee did not comply with the section cited above. LPA observed that Licensee and one assistant did not have a valid Mandated Reporter Training Certificate on file which poses a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee and Assistant are to complete Mandated Reporter Training and submit valid completion certificates to the Department.
