DOMINGUEZ, ELODINA FAMILY CHILD CARE

Based on observation, the licensee did not comply with the section cited above in as LPA observed 7 (seven) pickets that are loose from the fence line. There are gaps in the fence line and nails protruding out of the some of the pickets. The play yard toys have cobwebs and dirt which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to repair fence or replace fence pickets, remove or nail down protruding nails in fence and close gaps in fence line. Licensee agrees to clean play yard toys and inspect toys before play times. Licensee to provide proof of repairs to LPA on or before POC due date.

Based on record review, the licensee did not comply with the section cited above, LPA reviewed 3 out of 3 staff files did not have Mandated Reporter Training Certificates on file which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agrees to conduct Mandated Reporter Training and ensure staff completes Mandated Reporter Training, Licensee will provide proof of completion to LPA on or before POC due date.

Based on record review, the licensee did not comply with the section cited above. Licensee could not provide evidence of the above required immunization. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she has a scheduled Dr. appointment this month and will inquire with her physician if she can obtain the required immunization or received a letter of exemption. Licensee will submit proof to the Fresno licensing office of either proof of immunization or an exemption letter signed by her physician by 8/31/2023.
