DIAZ, CLEMENTINA FAMILY CHILD CARE

Based on LPA observation, the licensee did not comply with the section cited above. LPA observed observed front yard to have accessible garden tools, dog toys, and dog feces in the grass which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee is to move garden tools to an inaccessible area, to remove the dog feces, and to have pet items inaccessible to children.

Based on observation the licensee did not comply with the section cited above. Today, inside the day care room, LPA observed vitamins, a lighter, eye drops and batteries on top of an adult desk. In the accessible restroom there was glass cleaner, pet deoderizer under the accessible sink. Outisde in the play yard were accessible pruners and an unlocked shed that had a gas can. This poses a potential risk to the health, safety or personal rights risk to persons in care.

Resolution: Today licensee made all of the items listed above off limits. LPA reminded licensee to inspect her facility daily for any potentially hazerdous items inside and outside where children have access. Deficiency cleared today.

Based on record review, the licensee did not comply with the section cited above. During file review, LPA observed that 4 out of 5 files (child #2, child #3, child #4, child #5) did not have immunization records on file. Licensee stated they were in the facility but could not find them today. This poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she would find the immunization records or would request the current immunizations from parents and submit proof for children #2-#5 to the Department by POC date: 9/19/2023.
