Based on observation, the licensee did not comply with the section cited above as LPA observed ... items listed in LIC809.... which are accessible to the children which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will install child safety devices in kichen or make kitchen inaccessible to children in care by POC due date. Licensee will send pictures of kitchen locking devices to LPA by POC due date. Licensee stated she will remove and place all cleaning products and hair products inaccessible to children in care. LPA verified Licensee removed chef knife, lighters, cleaners and hair products during inspection.

Based on observation, the licensee did not comply with the section cited above in LPA observed through record reviews that four out of four staff files did not have LIC9052 which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will have staff completed LIC9052 and send copies to LPA by POC.

Based on record review, the licensee did not comply with the section cited above as LPA observed through record review two out of four staff files did not have immunizations filed and four out of four staff files did not have influenza declaration statement or influenza immunization which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee stated she will obtain copies of immunization records and Influenza declaration or influenza immunization for self and staff and submit copies to LPA on POC due date.
