Based on record review and interview, the licensee did not comply with the section cited above. The facility has not completed the required water testing for lead contamination levels. Water lead testing was to be completed by January 1, 2023.
PERSONAL RIGHTS- The licensee shall ensure that each child is accorded the following personal rights: To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. This requirement was not met as evidence by observation. LPA observed 4 of 69 children present not wearing mask while indoors.
