Resolution: Provide proof of mandated reporter training for all employees, assistants of the day care no later than the POC date of 5/15/2023.
Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in one out of two Assistants (A2) did not have the required LIC 9052 form in the staff files which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Provide a copy of the LIC 9052 form to the LPA no later than POC date of 5/11/2023.
Based on [(observation) (interview) (record review)], the licensee did not comply with the section cited above in one out of two Assistants (A2 )did not have the required immunizations which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee must provide proof of immunizations to LPA and/or the Department no later than the POC date of 5/15/2023.
