Resolution: Per Licensee they will contact the parent and the parent will contact the doctor and obtain a copy and once the copy is received by the provider, they will submit a copy to Licensing by agreed POC date.

Resolution: Per Licensee they will have the representatives provide a copy to her and she will provide a copy to LIcensing by the POC date and Per Licensee they will not allow the staff to return to the daycare until they provide their immunizations and will put it in a written declaration that is what they will do.
