Based on observation, interview, and record review the licensee did not comply with the section cited above in one child (C5) did not have their immunization records inside their file during inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee they will contact the parent and the parent will contact the doctor and obtain a copy and once the copy is received by the provider, they will submit a copy to Licensing by agreed POC date.

Based on observation, interview, record review, the licensee did not comply with the section cited above two out of two representative (S3 and S4) did not have their immunization records inside their files during inspection which poses/ a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee they will have the representatives provide a copy to her and she will provide a copy to LIcensing by the POC date and Per Licensee they will not allow the staff to return to the daycare until they provide their immunizations and will put it in a written declaration that is what they will do.

Based on observation, interview, record review, the licensee did not comply with the section cited above in one infant in care (C5) did not have a napping log during inspection in their files which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee they will begin a napping log as of today and continue to use it and provide proof to Licensing by Poc date.

Based on observation, interview, record review, the licensee did not comply with the section cited above in one infant in care (C5) did not have the LIC 9227 form signed by their parents and inside their file during the inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per Licensee they will have the parent complete the form at pick up today and provide the form to Licensing once it is completed by POC date.

Based on observation and record review, the licensee did not comply with the section cited above in two out of two staff S2 and S3 did not have CPR proof present during inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per assistant, proof of CPR training will be provided to Licensing by POC date of 10/3/23

Based on observation and record review the licensee did not comply with the section cited above in two out of two staff S2 and S3 did not have files to view during inspection which poses a potential health, safety or personal rights risk to persons in care.

Based on observation and record review the licensee did not comply with the section cited above in two out of two staff S2 and S3 did not have current immunization during inspection which poses a potential health, safety or personal rights risk to persons in care.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in LPA observed assistant providing care. Per Licensee adult is not associated with the home and proof of criminal clearance not provided during inspection which poses an immediate health, safety or personal rights risk to persons in care.

Resolution: Licencee will provide proof criminal background clearance for adult assistant by POC date of 9/27/23.

Based on observation and record review, the licensee did not comply with the section cited above in roster is not updated with the names of the children in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Per assistant, Licensee will update the roster and provide proof to licensing by POC date of 10/10/23

Based on observation record review the licensee did not comply with the section cited above in twelve children = C1-C12 did not have form LIC 282 in their files which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per assistant, form LIC 282 will be placed in children's file and proof of the form placed will be provided to Licensing by POC date of 10/10/23

Based on observation and record review, the licensee did not comply with the section cited above in eight of the day care children that were enrolled and newly enrolled = C1- C12 did not have the LIC 9224 form in their files for a previous TYPE A deficiency cited 5/17/23, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will let each parent know of any TYPE A deficiencies and provide a LIC 9224 form in child's files for all TYPE A deficiencies cited and provide proof to Licensing of previous Type A by POC date of 10/10/23.

Based on observation and record review, the licensee did not comply with the section cited above in twelve of the children's files = C1-C12 did not have proof of immunization records inside children's files during inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per assistant, Children's immunization records will be provided to Licensing by POC date of 10/26/23

Based on observation and record review, the licensee did not comply with the section cited above there was no current fire and earthquake drill during inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per Assistant conduct drill and provide documentation to Licensing by POC date of 9/29/23.

Based on observation, interview, the licensee did not comply with the section cited above in the licensee was not home during inspection and did not notify Licensing of absence or time frame which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per assistant, Licensee will notify Licensing of future absences and provide a written declaration to Licensing that this will be met by POC date of 9/29/23.

Based on observation, interview, and record review the licensee did not comply with the section cited above in three out of three staff S1, S2, S3 did not have the current training during inspection which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Per assistant, proof of Mandated Reporter training will be provided to Licensing by POC date of 10/3/23
