Based on record review, the facility did not comply with the section cited above by failing to provide the licensee’s current mandated reporter training which posed a potential Health, Safety or Personal Rights risk to persons in care.

Resolution: The licensee will email a copy of her mandated reporter training certificates to LPA.

Based on record review, the facility did not comply with the section cited above by failing to provide the assistant’s #1, #2, and #3 Pertussis and Measles vaccinations, including the TB clearance for Assistant #2 and #3, which posed a potential Health, Safety or Personal Rights risk to persons in care.

Resolution: The licensee will email copies of the assistant’s #1, #2, and #3 Pertussis and Measles vaccination, and TB clearance for assistants # 2 and #3 to LPA.

Based on record review, the facility did not comply with the section cited above by failing to provide the licensee and Assistants #1, #2, and #3 pediatric CPR/First Aid training which posed a potential Health, Safety or Personal Rights risk to persons in care.

Resolution: The licensee will email copies of her and 3 assistants pediatric CPR/First Aid training certificate to LPA.

Based on observation,interview, record review, the licensee did not comply with the section cited above. The licensee did not ensure that the body wash, conditioner and or other hazordous items were made inaccesesible under the cabinet which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: The licensee will lock will ensure the cabinet remains lock at all times when children are care

Based on record review, the licensee did not comply with the section cited above. Licensee's CPR/First Aid expired 07/2023, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will retake her CPR/First aid and send proof of completion to LPA Tamayo no later than 11/01/23.

Based on observation, record review, and interview the licensee did not comply with the section cited above. LPA observed the 3 car garage converted into a child care room without notifiying the Department, which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee shall obtain a Permit from Building and Grounds for the converted garage before children have access to the converted garage. Licensee is aware the garage will be off-limits until then.
