Based on observation, interview, and record review there was a seven month infant observed on their side without a signed LIC 9227 form, Section C by their representative to indicate if the infant could roll from back to side which poses an immediate health, safety, or personal rights risk to the person in care.

(a) Prior to the infant's first day at the center, the infant care center director or assistant director shall complete a needs and services plan for the infant.

Based on interview the facility was unable to provide a children's file for Child #4, which poses a potential Health, Safety or Personal Rights risk to children in care.

Based on LPAs observations and interviews staff do not treat children in care with dignity and handle children in a rough manner.

Based on observation, interview, and record review, the licensee did not comply with the section cited above in Staff 2 did not have their immunization records inside their files which poses a potential health, safety or personal rights risk to persons in care.

. Based on observation there were 4 infants sleeping with blankets in their crib, which poses a potential Health, Safety or Personal Rights risk to children in care.
