Based on record review, the licensee did not comply with the section cited above in not having proof of MMR,and assistant not having proof of MMR, TDAP and Influenza which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee stated will make an appointment for MMR, and will inform assistant to schedule an appointment with doctor to obtain a copy of immunization records.
Based on observation, interview and record review, the licensee did not comply with the section cited. LPA reviewed infant file and LIC9227 was missing form LIC9227 which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will send proof of form filled out and signed to LPA Beneroso no later than 08/31/2023
Based on LPA's observation, the licensee did not comply with the section cited above. LPA observed infant being unattended and without supervision in the day care room which poses an immediate health, safety or personal rights risk to persons in care.
Resolution: Duringn inspection, Licensee assigned one of the two assistants present to the infant to provide care and supervision.
Based on observation, the licensee did not comply with the section cited above. LPA observed all bedrooms (off-limits) of the home with open doorswhich poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: LPA observed licensee place a child safety gate in the hallway, making the all bedrooms inaccesible to children.
Based on observation, interview, record review, the licensee and her assistant did not comply with the Mandated Reporter Training which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee and her assistant will take the Mandated Reporter Training by 12/01/2021.
