Based on observation, interview and record review, the licensee did not comply with the section cited above in. Licensee's assistant did not complete her Health and Safety Training which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will send pictures of Health and Safety Training certificate or confirmation of enrollment in a class and assistant's complete file to LPA Beneroso.
Based on observation, interview and record review, the licensee did not comply with the section cited above in. There were two children's files missing which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: Licensee will send pictures of complete children's files to LPA Beneroso by 05/26/2023
Based on observation,interview,record review, the licensee did not comply with the section cited above. LPAs observed the babe was sleeping inside the crib with blanket on her. The licensee also used the blanket as crib sheet which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee removed the blanket and hanging object. She also put the new crib sheet on the crib.
Based on observation, interview, record review, the licensee did not comply with the section cited above. LPAs observed licensee and her husband both Mandated reporter trainings were expired which poses/posed a potential health, safety or personal rights risk to persons in care.
Resolution: The licensee will email the LPA her Mandated reporter training certification by 10/11/2021.
