Based on observation, the licensee did not comply with the section cited above health and beauty items on bathroom counter and in bath tub area which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will make health and hygien items inaccessible to children in care by placing in high, closed cabinets.

Based on observation, the licensee did not comply with the section cited above poisons, knives, razors, and medications stored in open unlocked areas accessible to children in care which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will remove poisons, knives, medications and other dangerous items immediately. Licensee will relocate items to a locked and or off limits area.

Based on observation, the licensee did not comply with the section cited above two baby bouncers in use during daycare hours which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will remove bouncers immediately from family daycare home. Licensee will no longer utilize prohibited items inside the daycare.

Based on observation, the licensee did not comply with the section cited above blankets and stuff toys housed inside the infant crib which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee removed items during today's visit.

Based on observation, the licensee did not comply with the section cited above soft book attached to side of crib which poses/posed a potential health, safety or personal rights risk to persons in care.

Based on record review, the licensee did not comply with the section cited above in physical checks documented every 30 minutes which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete and document physical checks on sleeping infants every 15 mintes as per regulation beginning 7/01/25.

Based on record review, the licensee did not comply with the section cited above per sleeping chart there is no documentation of infants up to 12 months sleeping positions which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will document sleeping positions of all infants including infants 12 months and under as they sleep daily beginning 7/01/25.

Based on record review, the licensee did not comply with the section cited above licensee and staff are in need of completing mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and staff will complete mandated reporter training and provide certificate of completion to the Department for review. Licensee and staff will complete mandated reporter training every two years as per resquired.

Based on record review, the licensee did not comply with the section cited above personnel files are missing immunization records which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete personnel files to include immunization records for licensee and staff.

Based on record review, the licensee did not comply with the section cited above licensee is in need of updated pediatric 1st aid/cpr which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete pediatric 1st aid/cpr and send proof of completion to the department for review.

Based on record review, the licensee did not comply with the section cited above employee rights missing from emplyee files which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide LIC 9052 for all employees for signature and file. Licensee will provide completed document for review by the Department.

Based on record review, the licensee did not comply with the section cited above licensee and employee files missing required immunization records which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will complete staff files to include immunizations required by the department for review.

Based on record review, the licensee did not comply with the section cited above children files missing parent rights which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will provide LIC 995A to parents of all children enrolled to sign for file. Licensee will provide signed copy for review by Department.

Based on record review and interview with Licensee, the licensee did not comply with the section cited above in 1 out of 1, sleeping log was not recorded which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to complete sleeping log and keep in the infants file.

Based on record review and interview with Licensee, the licensee did not comply with the section cited above in 2 out of 2 mandeted reporter certificates were not updated which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee agreed to complete Mandated Reporter and send Certificate by POC date.

Based on record review, the licensee did not comply with the section cited above in 2 out of 2 Pediatric CPR/First Aid were expired which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee and staff agrees to complete Pediatric CPR/First Aid and send proof of completion to LPA by POC date.

Based on record review and interview statement from LIcensee, the licensee did not comply with the section cited above in 1 out of 1 LIC 9227 forms were not completed which poses/posed a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will place completed LIC 9227 form in infants file by POC date.
