Based on observation, the licensee did not comply with the section cited above in that Electrical plug missing casing in bedroom #1, kitchen cabinet and drawers need safety locks, which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee fix the electrical plug cover, install safety locks in the kitchen by POC date and will email photo proof to LPA.

This report is being amended to correct the code Based on record review, the licensee did not comply with the section cited above in that Brenda Arias De Parada was not associated to this facility and was hired 1/15/24 which poses a potential health, safety or personal rights risk to persons in care.

This is an amended report to correct the regulation code. Based on record review, the licensee did not comply with the section cited above in that although Brenda Arias De Parada did have finger print clearance for Assisted Caregiver Services, she did not have her finger prints checked through the child abuse cental index system which poses a potential health, safety or personal rights risk to persons in care.

Resolution: Licensee will have Staff get finger printed for childcare by 1/8/25. Staff will not work until clearance is updated. Licensee will send proof fingerprints were done via email by POC date.
