Based on interviews, and statements of admission provided by Staff #2, it was determined that indeed Child #1 was left in the bathroom when Staff #2 walked back to the classroom with the other children which poses an immediate health, safety, and personal rights risk to children in care.
Based on record review, Licensee did send Lic 624 of the incident reported on 6/25/24 to the department within the 7 days required by the department, which poses a potential health and safety risk to persons in care.
(a) The licensee shall ensure that personnel records are maintained on the licensee, administrator and each employee. Each personnel record shall contain the following information: (12) Tuberculosis test documents as specified in Section 101216(g).
(g) Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children shall be stored where inaccessible to children.
